What is All-on-4?

All-on-4 is a full-arch rehabilitation in which four titanium implants support a fixed prosthesis for the entire upper or lower jaw. The two front implants are placed vertically; the two rear implants are tilted between 30° and 45° to maximize contact with available bone and distribute chewing forces evenly. This angulation avoids the maxillary sinus and, in most cases, eliminates the need for bone grafting before surgery. Patients leave the clinic with a functional provisional prosthesis on the day of surgery. The permanent prosthesis — typically zirconia for long-term durability — is fitted once osseointegration is complete, usually 3 to 6 months later. All-on-4 is indicated for patients with full edentulism or multiple failing teeth across an arch. A cone-beam CT scan is standard before planning to assess residual bone density and volume. Smokers and patients with uncontrolled systemic conditions such as diabetes require additional evaluation before surgery, as these factors can impair osseointegration.
